The NNE-PTN, led by Qualidigm in collaboration with the New Hampshire Citizens Health Initiative at the University of New Hampshire Institute for Health Policy and Practice, and Vermont Program for Quality in Healthcare, Inc., and practice assessments completed by the New England Region Quality Improvement Network–Quality Improvement Organization, are funded through the Centers for Medicare and Medicaid Services’ Transforming Clinical Practice Initiative to provide technical assistance to support you, your practice, and your consumers in the transition to value-based payment.  Additionally, the NNE-PTN partners with Mingle Analytics to measure quality and prepare for MIPS.

Practices participating in the NNE-PTN get access to the following:

Practices participating in the NNE-PTN will:

  • Receive no-cost access to PQRS Solutions, a CMS-approved Physician Quality Reporting System (PQRS) vendor.
  • Maximize current CMS payment incentives (and avoid future penalties) by complying with the CMS Value Modifier and Merit-Based Incentive Payment System (MIPS).
  • Participate in a short baseline assessment of your readiness to succeed in today’s healthcare environment, conducted by NNE-PTN staff.
  • Create a specific plan to address your practice needs and priorities, with a focus on supporting areas for improving patient care, team functioning, and the financial health of your site.
  • Leverage practice systems to capture significant revenue dollars from underused CMS payment opportunities.
  • Access a range of in-person and web-based quality improvement educational support and tools, customized to fit your practice needs Collaborate and learn from other specialist, behavioral health, and primary care clinicians and practices.
  • Earn Continuing Medical Education (CME) and Part IV Maintenance of Certification (MOC) credit for participating in NNE-PTN learning activities.
