Approaches to Care Coordination in Maine focused on Families, Caregivers and Children, 0 through 8

This module is intended to introduce the variety of care coordination approaches that exist in Maine, individual and collective responsibilities, and functions that they provide. Viewing this module will help to lay a common framework for Care Coordinators across all sectors.

Learning Objectives:

At the end of this module, participants will:

  • Have a common understanding of the variations of Care Coordination in Maine, including titles, preparation, and focus areas
  • Understand the importance of developing a common framework, language and shared vision for Care Coordinators across all sectors
  • Understand the cross-sector roadmap which will enhance communication and coordination that truly benefits families and children, and promotes greater satisfaction by providers

Module Structure:

This module is self-paced and available to any user that registers with the QC Learning Lab. The estimated time to complete is approximately 90 minutes.

The module contains fourteen (14) sub-sections or units. Each unit may contain various elements, including hyperlinks to readings and web resources, video lessons, and reflection activities. Two of the units will consist of the pre- and post-assessments, which need to be completed to receive your certificate of attendance.
