Family Engagement, Education and Support: Opportunities and Obligations

These Modules have been planned carefully to provide all Care Coordinators with a better understanding of the landscape of care coordination in Maine, as well as the specifics of this important service for families/caregivers and children from the prenatal period through age 8. This module focuses on the individual components of care coordination and introduces the various competencies that Care Coordinators should be able to demonstrate.

Learning Objectives:   

At the end of this module, participants will:

  • Understand the competencies that Care Coordinators should demonstrate consistent with their responsibilities towards families, caregivers and children from the prenatal period through age 8.
  • Be aware of the essential competencies from a practice perspective.
  • Have knowledge of the basic skills and practices that form the foundation of effective, respectful care coordination.

Audience:

Anyone who coordinates care for children and families in Maine

Module Structure:

This module is self-paced and available to any user that registers with the Learning Lab. The estimated time to complete is approximately 75 minutes.

The module will contain thirteen (13) sub-sections or units. Each may contain various elements, including hyperlinks to readings and web resources, video lessons, and reflection activities. Two of the units will consist of the pre- and post-assessments, which need to be completed to receive your certificate of attendance.
