The NNE-PTN Basics (Orientation for new members)

Description
This learning module provides an overview of the workflow and timeline for practices and organizations enrolled in the Northern New England Practice Transformation (NNE-PTN) program. The module reviews the enrollment process, initial participation steps, starting your specific practice transformation plan, and ongoing support available to NNE-PTN participants.

Intended Audience
Any individuals in eligible ME, NH or VT healthcare practices a) interested in joining NNE-PTN, b) wait-listed for NNE-PTN, or c) new to NNE-PTN – either as an organization or as a new employee to an existing NNE-PTN participant site.

Module Structure
This learning module is self-paced and participants can work through the materials entirely at their own pace.

Using the Practice Plan Tool

Description
This learning module is intended for NNE-PTN Practice Facilitators and Quality Improvement Coaches from partner organizations. It provides an overview of the newly developed Practice Plan Tool. The Practice Plan Tool is designed to assist Practice Facilitators and QI Coaches with facilitating the transformation process. The Practice Plan Tool collects all essential information in one location and can be used to document transformation activities and meeting notes. It also produces a customizable summary document that can be shared with practices. The summary document lists milestones by phase and score and provides current score descriptions, associated TCPI drivers, and the specific actions required to improve the scores from 0 to 1, 1 to 2, and 2 to 3. The goal of this learning module is to provide Practice Facilitators and QI Coaches with the knowledge and skills needed to effectively use this tool with their practices.

On April 27, 2016, the Centers for Medicare and Medicaid Services issued a Notice of Proposed Rulemaking (NPRM) to put in place key parts of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA). This bipartisan legislation, replaces the flawed Sustainable Growth Rate formula by paying clinicians for the value and quality of care they provide. The proposed rule would make these changes through a single framework called the “Quality Payment Program”.
